Partners in Recovery is a referral-based, virtual substance use counseling service as a continuum of care once a client has been discharged from detox, residential, IOP or a PHP program. I also accept referrals from probation officers, lawyers or court-mandated counseling for continued accountability.
It is at the sole discretion of the owner of Evolve Counseling & Holistic Wellness, LLC if an individual meets the criteria for services. Please contact the office via the referral form below. See dropdown tab under Partners in Recovery above for assessments available and the associated fees. SUD counseling only referrals can be requested below. Please leave the your name and contact information as well as the client's name and phone number in the the space provided below. Insurance may be utilized for counseling services, but is not accepted for any of the assessments. See the Counseling tab for insurances that are accepted.
