Thank you for contacting me for a DOT SAP Assessment. I am a trained SAP assessor. Training was completed by Professional Training Center, Inc. to assess individuals referred for substance use assessments for the following DOT regulated areas: USCG, FAA, FRA, FSCMA, FTA and PHMSA. I am also a credentialed, licensed professional with CAADC credentials with MCBAP.
If you are an employer referring an employee, please make contact with my office. Assessment & Screenings can be completed via telehealth/remote or in-person at my office in East Lansing, Michigan.

The appointment will take 3-4 hours depending on your ability to complete the paperwork, assessments and interview process. Please plan accordingly. If you need assistance reading or interpreting the forms, please tell the assessor.

SAP/DOT Assessments are $700.00. This fee covers the initial assessments and screens, follow-up visit, communication with employer, and management of associated documentation. If you pay for an independent assessment and then are referred for an employer assessment or via the court, you may owe another fee. The fee is due at the time of booking and is non-refundable. The employer or court-ordered assessment may be the only findings admissible to a court. The charges for the employer or court-ordered assessment may be your responsibility to pay, but will be determined with the employer.
