SUD Assessments |
Comprehensive substance use services includes assessments commonly used in the industry. A comprehensive summary findings report will be provided upon completion. The assessments are a self-pay service.
Upon completion of the assessment process, a report or letter will be provided within a reasonable timeframe to the referred party.
SUD Assessments and screenings can be purchased ala cart (per piece) or as a full package:

Insurance is not accepted for these services.
Total due at time of scheduling
Insurance is accepted for individual counseling services.
- Clients are accepted for assessments by referral only via legal counsel or court-ordered.
- Assessments are done virtually from anywhere in Michigan.
- Assessment appointment may take up to 3 hours or more.
- Payment is due in full at time of booking.

- Optional Assessments:
- SASSI Assessment $150
- MAST Michigan Alcohol Screening Test $50
- DAST Drug Alcohol Screening Test $50
- HAM-D17– Hamilton Depression Rating Scale $50
- Required - Comprehensive summary report or letter $100
