HOW IT WORKS
COMPANY OBLIGATION:
You refer your employee to Evolve Counseling and Holistic Wellness, LLC. You contact me at 517-712-9736 or via email at [email protected] to verify that you referred an employee and are authorizing X number of visits. You provide an authorization number or complete a purchase order/agreement or sign a contract for services that I provide (downloadable below) and return it via email. I send a statement when services are rendered. If the need arises and additional services are needed, a request may be made for additional visits to be authorized, or referral to a community agency or provider. Each visit will be invoiced at $100.00 per 50 minutes.
Since you are employer-paid, you have the obligation to inform your employee of the terms of service and if this is a requirement to employee's continued employment. The employee will receive verification of visit from me to provide back to the employer. No other information is shared with the employer. Please see corresponding employer form regarding confidentiality below to be utilized on your end.
EMPLOYEE OBLIGATION:
Employee is responsible to scheduling their appointments, receiving their verification of visit, returning the verification of visit to the Human Resources, Personnel or boss for the visit to validate their attendance with their employer.
An employee is responsible for rescheduling their appointments once they are made with me. A notice will be sent to the employer that the appointment was cancelled, no-showed or rescheduled.
Employee is responsible for completing in-office intake forms.
